It means only, that friends can cause me a pain fastly and more effectively, than enemies. Because you are more alert in relation to the enemy and do not allow him to creep closer. The friend, on the other hand, knows your weak spots, and a wound caused by him, always heavier. I know it on personal experience. But friends refer to as friends because it is possible to forgive them.

I am not always sad, but hardly I can be always cheerful. Hardly it will be pertinent, when you are at the height of fight with the vicious enemy.

Why? The smile can discourage the enemy, assure him that you are not afraid of him.
